Local 1250 Members,

Any member who spoils their ballot, or who has not received a ballot by Friday, July 28, 2023, should
call UniLect Election Service’s toll-free number at 1-866-466-6455 to request a ballot.

Members should provide their name, address, phone number, union name, member ID/employee ID/or
last four digits of social security number, and reason for requesting a replacement ballot. Members may
also contact the DOL Election Supervisor at (213) 534-6405 or email [email protected] or
the AFGE Election Official at email [email protected]. Requests for replacement ballots must be
received no later than 12:00 p.m. on Friday, August 18, 2023. Please allow sufficient time to receive and
return a replacement ballot since postal delivery times can vary considerably.
